This Agreement, made today serves to confirm that I/we (Name of Client/s), have contracted Prime BabyTime Consulting, LLC (Doula) to provide Postpartum Doula service for us after the birth of our child. We, the undersigned, have read, understand, and agree to the following:
A. LIMITATION OF SERVICES
We understand that the Doula is not here to make decisions but to help us understand our options in caring for our child and ourselves. The Doula will provide us with informational, emotional and physical support only.
In-home care of the mother, newborn & family includes (but not limited to) assistance with:
o Postpartum comfort measures
o Teach/Model newborn care i.e. eating, feeding, bathing, temp taking, and swaddling
o Non-medical physical, emotional and informational support after the birth of your baby
o Light housekeeping so that mom does not feel so overwhelmed o Parenting information and provide assistance with learning to feed and take care of your baby o Practical support will include preparing simple meals and snacks, getting frequent drinks and answering the phone and door if desired
o Do mom’s and baby’s laundry
o Organize nursery items; keep “changing station” tidy
The tasks provided by the Doula each day will depend on the priorities discussed by all parties before beginning services and on your particular needs that day. The Doula will not perform any medical procedures nor give medical advice such as examining the mother/baby, or taking temperature, blood pressure checks or any other type of postpartum clinical care. The Doula does not do major housecleaning tasks such as mopping, washing windows, cleaning tubs, showers or toilets. The Doula will not transport clients or take over all care of the baby.
B. OBLIGATIONS OF THE POSTPARTUM DOULA
Services are to be provided to (Client) at the address. Prime BabyTime Consulting, LLC will be on time for the new mother’s care and stay the entire agreed upon time. Occasionally there are events which are unpredictable and may affect the dates previously agreed upon. If the Postpartum Doula (Prime BabyTime Consulting, LLC) is unable to come, she will inform the new mother or a designated representative of her family as soon as the Postpartum Doula knows she is unavailable. Prime BabyTime Consulting, LLC will always keep the Client’s parenting style in mind and not force her opinions on the Client(s). Prime BabyTime Consulting, LLC will be available by phone and e-mail during regular business hours when she is not with the parent during the period of employment by the Client(s).
C. CLIENT OBLIGATIONS
Client will communicate needs and preferences with the Doula (Prime BabyTime Consulting, LLC) verbally and/or in a written form. We will call our Doula (Prime BabyTime Consulting, LLC) once the baby is born so she can make arrangements to attend to our family. We will inform our Doula (Prime BabyTime Consulting, LLC) if we are the carriers of any infectious diseases (colds, flu, strep). We need to let Prime BabyTime Consulting, LLC know of any special needs or customs they may have during the postpartum period.
D. FAILURE TO PROVIDE SERVICES OR TERMINATION OF SERVICE
If the Doula is unavailable due to illness or other emergencies during the contract period, there will be no charge for the missed services, or the Doula will refund the fees for the missed services.
If either the Client or the Doula terminate this agreement up to two months before due date, the deposit will be returned. If cancellation is made less than two months before due date, the deposit will be forfeited. If the Doula terminates services, she will return the deposit in full. Either party must provide written notice of cancellation (e-mail is acceptable). Absent written notice of cancellation request on part of the Client will result in the obligation to pay the full agreement amount (deposit and balance) and no refund will be due.
If after the Doula’s first visit the client wishes to terminate this service agreement, the client must provide written notice of cancellation after Doula’s first visit (e-mail is acceptable). Upon receipt of the written notice, the deposit will be forfeited and no balance will be due.
E. FEES FOR SERVICES
A deposit of $100.00 will be due upon agreement of this contract. This deposit will be applied to the consultation and the first week of care. In the event that you choose not to utilize the Doula services the deposit will not be refunded (Please refer to section “D” of this agreement). Services are expected to begin around the date agreed upon. Payment to the Doula will be rendered weekly upon receipt of the invoice.
The Doula requires at least 48 hours of notice prior to beginning services.
This agreement is meant to be fair to both parties, so that all parties may feel secure in their mutual commitment. In signing this agreement/contract, you agree to pay for services on the days that are mentioned above. In return, the Doula commits to provide these services and not to accept future postpartum clients which would interfere with her ability to perform the scheduled services.
